Consumer Directed Personal Assistance Program
This Medicaid program provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living (ADLs) or skilled nursing services. Services can include any of the services provided by a personal care aide (home attendant), home health aide, or nurse.

Consumers 18 and older call NYIA at 855-665-6942. Please Have your Medicaid #, Social Security #, Date of Birth and Zip code to enter when prompted.
- How do I find the Consumer Directed Personal Assistance Program?
- Before a person :
- 18 years of age and over can receive services, they must have a community health assessment and clinical appointment completed by the New York Independent Assessor (NYIA) to determine whether the recipient can appropriately participate in CDPAP.
- under 18 years of age can receive services, their doctor must send a completed Physician's Order for Services to the local social services district, which then completes a community health assessment to determine whether the recipient can appropriately participate in CDPAP Please see application for here (Insert CDPAP form as a downloadable attachment).
